This form is used to communicate changes, corrections and/or updates to existing Communication Requests. This form will be shared with the entire CB Communication Team.
This form includes, but is not limited to:
- Changing date/time of activity
- Cancellation or postponement (TBD) of activity
- Other changes to activity (cost, location, etc.)
This form should NOT be used to change communication channels or frequency of communications. Please use the Communication Request Form and answer "YES" to the subsequent form question.
