Eileen Testa discovered Jotform Enterprise entirely by accident. Several years ago, during the COVID-19 pandemic, she completed a COVID symptom screening form on her phone before a hair appointment. Ninth Street Internal Medicine needed a better way to collect patient information to combat form fatigue. With Jotform Enterprise, the practice can collect and manage information efficiently, using customized forms that are tailored to each patient’s needs.
Ninth Street Internal Medicine uses Jotform Enterprise to send patients a variety of screening forms before appointments. The staff use a different form for the different types of appointments a patient can make. For example, patients over 65 receive a different screening form than those younger than 65.
The practice also uses Jotform Enterprise to send follow-up surveys after appointments to conduct further screening, collect driver’s license or health insurance information, and more. Its website hosts an array of patient forms, some created from scratch and some made using Jotform Enterprise form templates.
Ninth Street Internal Medicine also uses Jotform Enterprise for internal purposes, like creating surveys for events like holiday parties and evaluating job satisfaction among staff. “Staff members can be honest because responses are anonymous, and it’s really good to hear those things,” says Testa.
Tracking population health in the Philadelphia community
Ninth Street Internal Medicine also uses Jotform Enterprise to track and report on population health, which refers to the health outcomes of a group of individuals. Population health can be measured by gathering specific metrics on health outcomes to identify trends and solutions to common health problems.
One measure of population health is the percentage of people in a group who have annual wellness appointments and regular screenings for cancer and other diseases. With Jotform Enterprise, Testa can gather patients’ health histories to ensure that each patient is up to date on their checkups and screenings, even if they received care before visiting Ninth Street Internal Medicine.

For example, she can include a question in appointment screening forms about whether the patient has received an annual mammogram. If the person says they have, the form can ask where and when they received the screening, so Ninth Street Internal Medicine staff can request records from the previous provider. If a patient’s response indicates that they are due for screening, a Jotform Enterprise appointment form will be sent to them directly.
“It’s really helpful,” explains Testa. “Especially for colonoscopy screenings — it’s brilliant! Patients only have to get those once every 10 years or so. If I’ve just started seeing somebody two years ago, they’re a new patient to me. And if they had a colonoscopy seven years ago, I have no way of knowing that.”
Testa can also run a report in Jotform Enterprise to get a list of patients who have not had an annual appointment or regular screening to target for outreach.
Insurance companies provide an incentive payment to Testa and her staff if more than 80 percent of Ninth Street Internal Medicine patients maintain recurring wellness appointments.
“It makes life much easier for us to meet these metrics required by insurance companies to get incentive payments,” Testa says. “That’s where the money comes in, and Jotform Enterprise makes us money.”
Maximizing impact and spreading the word
Testa was so pleased with the results her team is getting with Jotform Enterprise that she’s encouraging her colleagues to try it. Recently, she spoke at a large gathering of professionals from healthcare practices and primary care offices. She cited the impact of Jotform Enterprise and even showcased an example of one of the practice’s Jotform Enterprise forms. She also shared statistics on how Ninth Street Internal Medicine has used Jotform Enterprise to keep patients up to date on health screenings.
“Most insurance companies need you to have your entire population at about 80–85 percent compliant with receiving regular cancer screenings,” says Testa. “We’re in the 90th percentile, so Jotform Enterprise is working for us.”
“When I tell people that,” she says, “suddenly, their faces drop, and they ask, ‘What was that platform’s name?’ We strive to share this with other people because it’s valuable and it’s easy — it’s incredibly easy.
