Now, let me help you with your question. As I have verified, your account is suspended due to Phishing. The 'Employee Travel Authorization Form' contains fields that are collecting password details. Please note that sensitive information is not allowed in Jotform.
To reactivate your account we would need to delete the offending fields on your forms. Let us know how you would like to proceed. Once we hear back from your we'll ba able to help you with this.
