My forms 'say' that they are disabled and can't be found. Don't know why. I have had no submissions, have only five forms.. I'm stumped!

We do not allow for the collection of usernames and/or passwords through form for any reason. This is to protect users filling out forms from potential phishing. I removed those fields from your "Registration Made Simple" form and activated your account. Your forms should be usable once again.
