You can change the width of the form from the Form Styles in the Form preferences. Please see the screencast below:

information in the form does not extend. Perhaps I was unclear. I need
the boxes after "name", etc to extend to 1440px. I am also looking for a
clear form button. I see the submit button, however, the client would
like to have a clear form button.

If you want to expand your form in its full width, inject the CSS codes from the given link below into your form.

For the clear button, you can enable it in your form's submit button properties.
