I see the issue as well when I checked the form https://www.jotformpro.com/form/62646706559972 in your website.
To fix, can you please add this CSS codes to your form.
#customFieldFrame_94{
width:350px !important;
}
It should increase the width of the widget to accommodate the width needed.
Let us know if this did not work.

This makes perfect sense and I added the custom css however it has no effect for some reason, I see the added css injected but it does not change the width...
