I have checked this form http://www.jotform.us/form/62367646869171 and when I submitted it I got this Thank You page:
I have checked the Thank You page in the Quiz App and I can see it is the same, there does not seem to be any custom logo added:
Please do note that all the changes need to be done on the Quiz App if you have the show quiz results on thank you page option enabled then it will be shown instead of the Thank You page set up in the Form Builder.

Thank you it finally all works! I have a couple more questions:
1. Do we have to include the first name and last name? Can we just have the email field?
2. How many questions can we have on the quiz? (is there a limit?)3. Do you have plans to add Shopify as a payment merchant in the future?4. Is there a way to get to Quiz App from the Jet Form platform?
If you could let me know, I would appreciate it!

Glad to know you were able to edit your custom thank you message.
Now regarding this:

Yes, you can simply remove the first and last name field, you can do remove those fields from your form, custom email and custom thank you page as well, the quiz app should still work fine.
