You can go to https://www.jotform.com/login and then utilize the "Forgot Password?" link.
I have checked the logs of the email address you include in your post and I see that the reset instruction was forwarded successfully. Kindly check your email address' Spam or Bulk folders and check the email that contains the reset instructions.
