The changes are automatically saved as soon as you check/uncheck the options in the submissions page settings. I've checked your form submissions page and the map is already unchecked and the texts are already shown. I think you've figured this out already.

How can I edit my form? The date comes on a sheet by itself. I need to move the date up so everything is in 2 pages.

I have checked your both the forms, and there is only one page in your both the forms. So please confirm, do you want to create multi page form, also please confirm which form you want to edit.
If you want to create multiple pages form, then please check the step by step instruction on below guide: https://www.jotform.com/help/119-How-to-Create-Multi-Page-Forms
In general here are the steps to edit your form:
1. Sign in to your JotForm account.
2. Click on the "My Forms" link from the header.
3. Select the form you want to edit and then click on the "Edit form" button. Please check the screenshot below:
