Hi,
Appreciate that you can find out what is the problem that our form appear an error " Form Quota over". This form is important to us and patient book in the appointment. Kindly see the printscreen attached.

It seems your account was downgraded due to reach your form views limit. To solve your issue I suggest you sign up by the help of the link below and upgrade your account to desired plan.

As a free user, you have 1000 Form view limit per month and you used 1487 of form view limits. When you embed your form to your website and viewed by somebody, the counter will increase each time when your form is loaded. Once you reach the monthly limit, you don't receive more submissions until next month.
