My form disappeared. How do I recover it?

I was deleting old submissions and when I tried to edit my form I discovered it was deleted with the submissions.

How can I recover it and how did it disappear?

If the form was purged from the trash, it is fully deleted from our servers and cannot be recovered.  If you have the URL of the form or a page where the form is embedded, we will be happy to see if it can be recovered.
