While browsing our site in Chrome, an alert from McAfee popped up indicating that it blocked an IP from interacting with my browser. Looking at my site, I realized that the form didn't load, so it looks like is what was blocked.
Using Developer tools, I did see these errors;
tag_assistant_compiled.js:117 GET https://form.jotform.com/jsform/60824920551958 net::ERR_NETWORK_ACCESS_DENIED
also
jquery.min.js:2 Uncaught SecurityError: Failed to read the 'contentDocument' property from 'HTMLIFrameElement': Blocked a frame with origin "https://www.foremostequipment.com" from accessing a frame with origin "https://www.google.com". Protocols, domains, and ports must match.
The IP that mcafee says it blocked is 45.63.78.200

Your form seems to be functioning fine from here, though I'm not running McAfee. Here is how your website appears from our end:
The IP address 45.63.78.200 belongs to JotForm.
I have used VirusTotal and tested the form URL that was blocked in your case, and the result of the analysis was that it wasn't being blocked by any internet security labs:
As anti-virus providers update their blocklists and whitelists several times a day, it may have been a temporary false positive (when a good resource is accidentally blocked). I would recommend updating your McAfee virus database, and check your website once again to see whether the form is working correctly.
If McAfee is still blocking your form from loading even with the updated virus list, it would be best to contact McAfee directly so they can make sure that your form found behind IP 45.63.78.200 is not blocked.
You can also add an exception for this IP address to your McAfee program to allow your form to load. Please let us know how it goes, and whether you continue having any difficulties afterwards. Thanks

now it shows good. so you're right, it may have been temporary.
It's still not showing the form for me and I couldn't find where to allow
the exception in LiveSafe, but I'm thinking when i reboot it will be ok.
The important thing is that it's not happening for everyone.
