Please provide us more details about your issue. Is your form not shown on mobile devices or it is not responsive? If you form is not responsive, you can add Mobile Responsive widget to your form to make it responsive.
How to make mobile friendly forms on JotForm
Also provide us ID of the form you are having issues with. In case that your form is embedded, please provide us URL of your webpage.

I have tested both URLs on my Samsung Galaxy S4 and I was able to replicate mentioned issue. However on the bottom of both pages I saw that there is Mobile and Web option and Web option is selected by default.
On first webpage I was not able to switch to Mobile. Most likely there are some script conflicts between your webpage scripts and JotForm scripts. On first webpage you have two embedded forms both with script code and one of them is deleted. I would suggest you to delete script code of the form that was deleted and to replace script code of your other form with iFrame code, to see if that will resolve your issue. On second URL I was able to switch to Mobile view and your form is shown properly.
I would also suggest you to re-embed that form with iFrame code.

I just looked into it... and it doesnt work. Ive deleted and copied a new embed code... its the same.
also, I believe that it does (a least on my iPhone) defalts to mobile, and the word WEB is highlighted to let you know to click it. Ive clicked web and it works.
on the 2nd webpage (FGpizza) it is the same.
i guess it just doesnt work on mobile.

I did some additional tests and form is shown only when Web option is selected.
However when I inspected your forms again I saw that your form are still embedded with script code.
