My form has successfully loaded into FB business page. However, the form's visual resolution is lousy...choppy & rough. Am I doing something wrong or is there a fix?

Could you please provide us the Facebook page where your form is or a screenshot for us to better understand the problem? It would be quite hard for us to come up with a resolution without actually seeing the problem.

If you are replying here using your email client and you are attaching images or files, it won't go through here. Please upload the file, image or screenshot on a hosting website like Google Drive or Dropbox, then share us the link so that we can check it.
If it's an image, then you can just directly view this thread using this link https://www.jotform.com/answers/883568. Then upload the image using the image upload in the text editor:
