I've already created a form via JotForm. When someone completes this form, I would like the form data to be integrated into my MailChimp subscriber list. I created an API key and tried to authenticate it but the authentication failed.

If I understand your issue correct you are unable to integrate with mailchimp because the api authentification on your form failed.
I have tested this on my end but it is working in a brand new test with my own api key.
Is this http://www.jotform.us/form/61893221184153 the form you are trying to use it on?
Can you try it again? Perhaps there was a temporary problem and let us know.

Yes, that is the form I am trying to integrate. I've attached images of what I have done and the failed authentication.te with MailChimp.

Please create a new API key, the integration is working. I just tested it in my end with a new API key I just created:
And it worked:

1. Create Form
2. Go to My Account - Click on API in left menu
3. Click Create New Key
4. Copy newly created API Key
5. Go back to the form created in step 1
6. Edit form and click on Integrations in the top menu
7. Select MailChimp from the integrations list
8. Paste the API Key I copied in step 4
9. Click Authenticate
10. At this point I receive the authentication failed message
Am I missing a step or doing something incorrectly?

Looks like you are using a JotForm API Key which is not right.
You should use your MailChimp API Key instead of the JotForm API Key since you will be integrating with MailChimp.
Here's how to get your MailChimp API Key http://kb.mailchimp.com/integrations/api-integrations/about-api-keys.
That should now work.
