I am trying to generate a sales report and I was disabled for phishing. Please let me know how to adjust the form and reactivate my account; I am not asking for sensitive information.

There were various fields in your revenue report form that set off our phishing detector. References to credit cards and things of that nature tend to cause immediate suspensions. I checked your form and it does look to be within our terms of use. I whitelisted your form and activated your account. You should no longer have any trouble with that form and your account is enabled once again.
