Sorry, I wasn't sure to post the question..
Here's my question about Auto fill in Addresses..... if I created dropdown list of customer .. How can I do IF ... DO condition to fill in Address ...

that's easy to fill in with only one field ... not like addresses have multiple field and I couldn't do the IF condition to auto fill in ...
... example.. I'd like type in Lab Name: Verch .. then it will fill in Address, Phone, Email ...

Hi,
If you want to fill multiple fields via condition base on a single IF rule, then you have to create multiple conditions for that.
Here's an example:
Please note that you have to define all the values into the conditions wizard. If you're looking for retrieving these values somewhere else (like from a database) and populate them into the fields, then that's not possible in the conditions wizard.

Unfortunately, there is no a way to get data from a database and insert it into your form fields, to do this you will need to get the source code if pre-populate the fields via the URL, here is a guide about pre-populating fields: Prepopulating-Fields-to-Your-JotForm-via-URL-Parameters
But there is no an integration or app that will help you to get data from a database and insert it into forms.
