I have checked your account and indeed it is suspended, this happens when you're asking for info that is not allowed at JotForm, then our anti-phishing system detects this activity and proceeds to suspend the account.
However, I have checked your forms and I can see they are correct, you do not seem to be asking for info not allowed, I would like to share you the link to our terms, so you will know what info you can and you cannot ask in your forms: http://www.jotform.com/terms/
I have re-activated your forms and you should be able to log into your account now, please check and confirm us that you were able.
