The second form you are referring to seems to have been archived (http://www.jotform.com/form/61026284421144). Do you want to add product dropdown options to another different form? To do so, simply follow this guide:
1. Click the wand icon to open the payment wizard.
2. Then while on the payment wizard, click Next until you get to the Products.
3. Hover on the product you want to modify and click the "Add New Option" button.
4. Select "Add a custom property" and click next.
5. Input testname as the Name and input the text below in the Properties:
(Please Select)
Supporter--$300
Sponsor--$500
Patron--$1000
Benefactor--$5000
6. Click Finish and Save Product. Click Next again until you reach the Integration Complete page.
Your product should now have the dropdown options as seen below.
Then you can inject the custom css below to make the edges more rounded:
#input_13_custom_1019_0 {padding: 3px;height: 30px;border-radius: 6px;margin-left: 0;border: 1px solid #ccc;font-size: 1em;line-height: 1.071em;-moz-box-shadow: 0 1px 2px #eee inset;-webkit-box-shadow: 0 1px 2px #eee inset;box-shadow: 0 1px 2px #eee inset;}
If you need further assistance, please let us know.
Regards.
