You previously have a form that captures username/password.
this was the reason our Phishing filter auto suspended your account and disbaled your forms.
Since you have now deleted the bad form, I have resolve the issue and reactivated the good form and your account. You may now continue using JotForm services.
Let us know if there are further concern.
