That is already included in the notification, if it does not show, simply update the notification by deleting it and creating a new one: http://www.jotform.com/help/25-Setting-Up-Email-Notifications, as well it will appear in the CSV file: https://www.jotform.com/help/44-How-to-Export-Form-Data-to-Excel
