If you want to print your form, you will need to add Print button to your form. Inside of this guide you can find how to add print button to your form:

Then you would also be able to print a form exactly as it is when viewed, but with the user entered data: http://www.jotform.com/help/335-How-to-print-a-form-exactly-as-it-is-when-viewed-but-with-the-user-entered-data

When WE receive a filled in form, we want to print it out so it looks like a PDF version of the form (with fields, graphics etc.) I don’t care if a sender prints it out at all.

If you want that images and text are shown inside of PDF report, then you need to select show headers and text option on submission page of your form. I saw that you already did this on your form.
However if you want to print submissions of your form which will look exactly the same like your form, then please check guide I previously provided: http://www.jotform.com/help/335-How-to-print-a-form-exactly-as-it-is-when-viewed-but-with-the-user-entered-data
When you print your submission, your form will look like this.

The PDF headers and plain text look like the online form, but the rest of fields do not. The president of my company is unwilling to have members/customers use this online form unless the submissions look like the screenshot. He doesn’t like way the PDF field information looks.
I’m good with it, but he’s stuck in his ways.

The only way to have the form print exactly as it looks in the form is to add a print button to the form and print the submission data directly from the form:

When a submission is edited view the submission view or view an edit link:
http://www.jotform.com/help/40-How-to-Let-Users-Update-Their-Form-Submissions-at-a-Later-Date
The full form will be shown and can be printed as is. Other than that, we do not have the ability to make the PDF look exactly like the form.
