I need to build an online registration form for our local Junior Cricket club.
My current form is here:

Everything is fine if each parent only needed to register only one child.
However some parents need to register more than one child.
Hence, I have included a field "Do you want to register a second child?" third child, forth child.
The problem:
When more than one child is registered it creates both child's details on one line in the database, Excel spreadsheet.
This is very difficult and messy to deal with and does not display in a usable format in an HTML Table Listing.
I would like the data for each child to be on a separate line.
However I do not want the parent to have to complete a new form for each child.
So if more than one child is registered how do I build the form to write the data for the first child on Submit and if there is a second child, continue the form and write that data on to a new line on the next Submit etc?
Any ideas?

I've had an idea!
If I create a Start Form and this redirects to a second form for the second child's details if the condition for "Register another Child" is Yes or redirects to the End Form for payment if NO.
So I will have a:
START FORM - Parents details & Child 1 details.
CHILD 2 FORM - Child 2 details.
CHILD 3 FORM - Child 3 details.
CHILD 4 FORM - Child 4 details.
END FORM - Final details & payment.
This may work?
