I am still not quite where I need to be with this form.
I can't seem to get more than one line of text above each calculable slider.
It needs to look like the image attached.

How about using the single slider widget and add it below the html text field? I think this is pretty much easier.
Alternatively, if you know basic html codes, you can apply it on the "sliders with calculated result" widget to create a format base on your example.
Use:
br tag to create a new line
b tag to create bold text
i for italic
font to add colors on the font.
