You can embed videos into your form using our video widgets, example youtube. Is this what you mean?

Also, I don't think embedding and HTML or javascript code is supported on powerpoint since this is an app for presentations. However, it is possible to embed your form on facebook pages. Guide http://www.jotform.com/help/22-Adding-Form-to-Facebook

We do have Iframe code, but I do not think it will be supported by PPT: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
