I created a new form and it has been saved. I copied the embed codes to my webpage, but the old form shows up and when submit is clicked it doesn't work.

Hi,
I checked your site and I noticed that your form is embedded using the script embed. Can you try re-embedding your form using the iFrame embed code and see if the issue still persists? Here is an article that may help you with getting the form iFrame code: How to Get the iFrame Code

I used the iframe code and now it displays correctly, but when it is submitted it will says the "form is unavailable".

I checked your website and it is still using the default script embed code. Please remove it first and get a fresh copy of the iFrame in your "Publish" wizard. Here's a guide on how to get your iFrame embed code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
I also tried filling out your form and clicked the submit button, I got the "Thank You" message but the problem I saw is that it did not scroll to the top part of the page.
I'm unable to get the error message "Form is unavailable".
Please let us know if the iFrame embed code works.
