I took a look at the email logs and it seems that you were sent emails. I have checked all 3 emails that were used - for the email and your account and all seem to be receiving emails.
If you are not receiving them, please do check your trash and spam folders to make sure that the emails are not ending up in there.
If you find them there, do mark them as not spam.

Thanks for the response. However, I have checked the spam/junk folders on
both email accounts ( info@kasloshops.com and s_black@live.com), and I do
not see either of the responses. Are those the two emails that they were
sent to?

Hi Steve,
I checked your form - http://www.jotform.ca/form/53418894863268 and the notification email of the form is sent to info@kalsoshops.ca.
I checked the mail log of the form and the emails are successfully sent.
You can also check your form's Email History on your end, just follow the instructions provided in this article: How to view all your form Email History.
