please find my web page & attached file as below cos i don't know why cannot show the answer for my email respond. i think the problem is i set other function (am i right?)

I have inspected your email Notification settings and it seems that all fields and values of those fields are set correctly. However I was able to replicate mentioned issue on my cloned form.
It seems that you did not set your conditional logic correctly and because of that values of mentioned fields are not shown inside of you email Notification. When I disabled all conditions on my cloned form all submitted data were shown inside of my email Notification.
I would suggest you to check your conditional logic setting for those fields.

done, but i think the problem is the other field, cos when i close the other field then will be OK

Did the solution suggested by my colleague help you resolve the issue?
I'm a bit lost as to which field you are referring to is it the one showing/hiding the passport radio-buttons?

for passport radio button, when i open "other" for guest to select. then cannot show the selection on submission. and when i close the "other" then will Ok to show the selection on submission. so please help any problem on this. is it any miss for my setting

Hi Jason,
Could you be referring to the other option in the radio button passport type list.
I tried sending with the option check and managed to receive the data in the email.
Is it possible to provide screenshot highlighting the behavior you are experiencing on your end so that we may be able to advise accordingly.

i means when i open the "other" function, then select another button, for example, "Australia". but submission cannot show "Australia".
but when i close "other" function. then will be OK. the problem on this.

I was able to recreate your issue, when you choose "other" then select the drop-down value, no value is passed to the email notification as well as in the submissions collections.
I do not know what is the cause of this behavior for that particular radio-button group as I have tried recreating it with a fresh set of radio-buttons but I have not been able to, a workaround will be to add another entry called "Other" then using conditions when this option is selected show a hidden text field an also set is attribute to required.

From what I have investigated, it seems the issue was due to the not updated Autoresponder Email of the form http://www.jotform.me/form/53161507184452
Please try these steps to refresh the Autoresponder Email
#1 Allow Other enabled in the Radio button
#2 Save the Form
#3 Redo the Autoresponder by adding a new Autoresponder Email for the form
#4 Save the Form.
Take note as well that in Preferrences / Advance Settings -- the 'Clear on Submit' for Hidden Fields is enabled. Try disabling this.
Save the form. Then, test submit the form to check if the new Autoresponder now includes the data from 'Other' option of the Radio button.
I did have a test form https://form.jotform.com/53498538508974? that have the setup. It was working as intended. If you test my form, provide your email address and choose the 'Other' option and input any text.
The resulting auto email you will receive should contain the text you have put in the 'Other' field.
Let us know if these process still did not resolve the issue.

You are welcome. Please do try my colleague's suggestion. Please recreate the autoresponder, and make sure that everything is setup properly. I checked it in my end and when "Allow Other" option is enabled, is should still properly fetch its value and add in the email autoresponder.
You can check this form: https://form.jotform.com/53500758326960. Enter your email address there and select an item in the radio field. You should receive an autoresponder with the data on it.
We'll wait for your response.
