Venho mais uma vez pedir ajuda por favor
meu formulário esta desconfigurado já tente varias forma e não funciona, o e-mail não chega segue imagem em anexo
tem um telefone que eu possa ligar

I am not sure if I have understood your question correctly. The image you have shared of "Image Checkbox" widget and you seems to have added the email address in the widget. All the image URL you have added in the widget does not seems to be valid. Please check the screenshot below:
Can you please check if the domain name you have mentioned in your image URL is valid?
Please explain your issue a bit more in detail and we will surely help you.

I checked the URL links of the images you used on the Image Checkboxes on your jotform http://www.jotformz.com/form/53442576164660 BUT they do not exist.
So the URL link to the images were incorrect or they do not exist.
Please check first the images using the URL if they will display properly on the browser.
Then re-create the Notification Email of your form so that all the fields from the form will be added to the Email.
