Above are two forms I would like to create but don't know how using your service. could you please have a loo and creat them for me or let me know how. kind regards

Hi Meera,
Jotform has many features, but creating the template of your form is really easy. All you need to do is drag and drop the items you like.
In your thread, you mentioned two forms those you want to recreate on JotForm. For cases such of this, we have clone tool to import webpages, but it doesn't work perfectly when the form comes from outsource. On the other hand, we can use it to create main structure of the form.
Below, you can see the image of form after cloning (Second form from your thread). As it is seen, some fields are wrong, some texts are missing; but all the fields with their values are there. So, we can continue to modify it from there.
To add heading to your form, you need to drag and drop the "Heading" field to your form, the copy and paste the text.
To add "Sub Label", select the related field and click to "Sub Label", then copy and paste the original text from your source form. Do this step for all fields those have sub label.
To change the fields title, select the related field and click to its title again. Copy and paste the text, then hit the enter.
