I want patient intake process documents on my website. Patients could complete and email to us. You were recommended by Therapy Sites. My 12 page doc is in Word of PDF. How can I set it up thru you to link to our site, patient completes, signs, and emails back to us? Thanks adrobbjr

You will have to create it in the form builder. After building the form, you will have to setup a notifier to be able to receive an email once someone submits the form. Here's how to create a notifier in the form:

You can also import the PDF to Jotform, but that will only give you the same PDF format so if you want an online version of the form, you will have to create it in the form builder.
