Hi,
Upon checking the form, I see that you are using the old field tag in your email notification.
In your email template, the old tag is {telephone}, if you check form field at the right and click the correct field, you'll see that the field tag should be used is {telephone27}.
This happens when you edit or delete a form field, you need to edit the email notification to make sure that the correct tags are used.
Or you can create a new notification to have all the field tags to be updated automatically.
