Apologies for the inconvenience.
It seems like the "Import Forms" button is not anymore available for use. I will forward this to our developers to have it checked and fixed. We will update you on this thread as soon as we hear any news.
For the meantime, if I may suggest, you could recreate the that PDF form using our form builder, that would allow you to fully utilize our features, apps and widgets. When you upload a PDF form, it won't be editable in our form builder. You can always convert the online form as a fillable PDF.

Import forms buttons is available now. You can import your PDF forms from the link below:
http://www.jotform.com/adobe-formscentral-alternative/
From there click on "Import Your Forms" button at the center of page or scroll down and click on "Import Forms" button at the upper right corner. Then click "Upload" button on PDF only option.
