    Hi,

I would recommend you to use our Image Tool for adding a logo to your form:

    Hope this helps.

One way to add a logo would be as follows:

    1. Go into the Designer:

    2. On the Design tab click Form Cover and to expand it and then replace the default image URL with that of your image:

    3. Save the form:

As to the image, if your logo is stored in your JotForm account, you can get its URL by first inserting the image into the form and then clicking the Image Source button:

Otherwise, just get the URL from wherever the image is stored online.

If you need further help with this or anything else JotForm related, we'd be glad to provide it.

    Cheers
