You mean to say you have received submissions in your form but no email notification went through to your inbox? I can see that your last edited form have 4 different email recipients. I would suggest to check the spam folder of these recipients or anything that could possibly block/flag the notification coming from Jotform. Perhaps adding Jotform to your email service provider's white list might help. http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses
