My form has been completely wiped out, and not disabled. Can you have a look please, and let me know what the issue is?!

Hi,
I checked your account and your forms appear to be in order. We were having an error that was causing fields to be stripped from forms. Most of which could be restored using the revision history:

If you could let us know which form you are having trouble with, we will pass it along to our developers and can also assist you with getting it restored.
