I'm trying to turn our client intake form into a jotform. I need a parent to click yes or no to each question but quite often on our written forms they jot a note in the margin that is quite helpful. Is there a way to force them to choose yes/no AND allow them the option to write a note?
Currently I have a checkbox field with yes/no/other with the other text saying "Add a Comment" however there is a possibility that a parent would just check "other" instead of yes/no AND "other"
Is there anyway to display an associated text field while still forcing a yes/no choice?

Hi,
You could just add another field for the comment box, just separate it on the check boxes.
Here's what I did:
1. Use the radio button and only add "Yes" or "No" to the options. Then add a "Text area" as the comment section. Make them both required so that the user would be force to fill those up.
