Hi Kim,
Thanks for reaching out to Jotform Support. We understand that this may not be the case with your form, but we must adhere to our Terms of Use, which state that collecting credit card information is not permitted in Jotform. I suggest contacting them directly to obtain such information. To reinstate the account, we must remove the credit card fields from the form.

Yes, I understand the information needs to be removed. But I am not able to login. I changed the password several times. My username is eaverhart and it still says my account is suspended. Who do I contact directly?

I can get into my account. Would you verify if this form is appropriate. I am not collecting payments, I am just getting authorization from the client in advance to charge their card with the supplier. I do not take payments.
