    Hi naturalwellnesscenterllc,

Thanks for reaching out to Jotform Support. You can use the QR Code Reader to get value from your QR Code and pull data from the Spreadsheet to Form widget. Let me show you how to do it:

    1. First on a separate form you can create a QR code with the specific message to encode. In the Form Builder, click on Add Form Element + menu and go to the Widgets Tab. Select QR code.

To create your Form with the QR code reader to fill up data from the Spreadsheet to Form widget follow the steps below:

    1. Search and add QR Code Reader and Spreadsheet to Form widgets to your form. Make sure you have uploaded your Sheet.
    2. Click on Add Form Element + menu again and go to the Basic tab.
    3. Add the form elements that you want to pull from your Spreadsheet.

    6. Go to Conditions, and click on Update/Calculate Field.
