Create form in sections

I have attached a photo of a form I wish to create an online form using jotfrom. 

I need to create the form so there are side by side sections as seen in the attached photo.

If you create the fields you can set them to go to left and to right column (be split) within the Form Designer - which is used to style the jotforms.

You could also use the shrink feature, but I believe that the first option would work better for you.

Now in terms of them having different background colors, that would be possible with CSS, but we would need to see the jotform in order to help you with the CSS and some final touches.

Do you have the jotform already created that should look like on the screenshot?
