The required field 'Full Name' is disappearing from Notification emails on our form, USAA. This field is visible if I pull an excel list of all submissions, but is not delivered via email notifications.
I've also attached an example of the email notification for reference.

Please do the following:
1) Delete your email notification:

2) Clear your forms cache:
https://www.jotform.com/help/229-How-to-Clear-your-Form-Cache
3) And set up your email notification again.
