Could you try embedding the JotForm using iframe method to your website? You can get the iframe code of your JotForm by following the instructions provided in the guide below :
http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Please let us know if the issue still persists after embedding with iframe method. We will be happy to look into this issue further.

Upon reviewing your form, I see it asks for password credentials, that is no allowed here according to our terms of use:
I have reactivated your account and removed that field, please make sure all your forms do not contain such a field, or violates our terms of use, otherwise, your account will be suspended again.
