I see you have several form, in which specific form you are trying to accomplish this?

I have tried to change the layout of the PDF in email settings but it does
not seem to work. I am trying to just use the downloadable PDF from
submissions but a few issues:
1:It becomes long because of single column answers.
2:I need certain information on every page ( My header, patients name)
When I get this form straightened out I will use the prepopulate app to
fill in certain information for each patient and then open their form each
visit. My biggest problem as I have said is getting the form results into
an acceptable presentation. The best would be if I could change the layout
of the PDF in email settings so that do not have to go to submissions each
time to download the results.

I also included a recent submissions report...I can live with this if I can
get my name and the patients name on every page. It is a bit long and I
would like to make it look a bit more professional

First, of all, very sorry for replying back to you till now. The system did not bring your response due to a problem.
On regards to willing to customize the PDF output, let me tell you this is not possible for now unfortunately. We have already a ticket opened for this feature request, hopefully it will be implemented in the near future.
