Platform Integration: How to add a form in Facebook Page?

Thank you for reaching out to Jotform Support. I understand that you want to create a form where it allows your users to fill out the form and then upload documents. Doing that is easy. Let me show you how:

• First, Create a form that contains all the fields you want your users to fill out.

After you created your form in the Form Builder, please follow these steps:

1. Click Publish in the top menu.
2. Go to the Platforms tab.
3. Select Facebook from the options.

1. Click the Add Jotform Tab button.

1. Assuming you’re logged in with Facebook, select the page from the dropdown and click the Add Page Tab button.

1. Go back to your Facebook page after successfully adding the tab.
2. On your Facebook Page, click More and you will see a Contact Us tab. Click it to view your form.
