I recently updated on of my existing jotforms with the form designer. Now the words "sample logo" and a circle with an S are showing up at the top of our form. This form is used heavily on a daily basis and the fact that "sample logo" is the first thing our users see is not appealing.
Please help resolve this issue as soon as possible.

Adding the following rule to your jotform's injected CSS will remove the logo:
.form-all:before {
background-image: none;
