When the submit button is pressed on the request form, it does not submit to my email address.

Hi Roy,
If you want to use custom sender emai in your form Notifier, then, you must setup first the SMTP email sender on your account.
OR use the 3rd party email authentication like Mandrill.

Just in case the 2 suggested method is not an option you can use, you can actually just leave the Reply-To-Email blank(none), and the email should still arrive in your mailbox.
