Hi,
That is done using custom injected CSS codes.

Using the new Form Designer, you can add icon/image to the input box(in this case the Textbox field) by going to the CSS designer and inject CSS code like this
sample CSS
#input_1 {
background : transparent url('http://www.yourwebsite.com/icons/name_icon.png') no-repeat 20px center;
background-size : 20px;
}
where #input_1 is the field ID#

I have also submitted this post to our higher team support as a feature request ticket to have the feature added in the Form Designer. The new feature is to have option to easily add icons/images into the input fields.
