How to I copy of newly revised URL to the Quick Share section and to also create the phone app?

1. In the Form Builder, click the Publish button.
2. Under the Quick Share tab, click the Copy Link button to copy your form URL.

when I do this the "yes"'s are left off of the form.

Thanks for getting back to us. In your other ticket 4490565 it sounds like you made a prefill link that these were prepopulated with the answer yes. If you don't use the prefill link that you created these won't have the values inserted into them already. You'll want to use the URL that you created in the prepopulate app to have them filled in.
