Currently, the form has separate entries for First, middle and last name. But in the Email alert, all three a presented together and it is sometimes difficult to know which names are part of the first, middle or last name (e.g. Benoit Steve Joseph Perry - not clear whether Steve and Joseph are part of the middle name, or first or last). Ideally, the Email Alert should be able to label each so it is clear which words part of first, middle or last part of the name.

Hi ststephenchelsea,
I cloned your form and I believed the email alert is setup properly. Your form is showing the First Name, Middle Name, and Last Name.
I filled it up and submit the form. I received an email alert and it shows the name separated properly.
On your email alerts, it is showing the proper field names.
You can identify the field name by clicking the Field > Settings > Show Properties
At the bottom you will see Field Details.
