Hello,
I would like to provide a direct link or downloadable link for the pdf version of my form, from inside my form.
This is because, we have a multi-page detailed form and it will be nice for the applicants to review the form before starting to fill it out.
So, for example I would have something like this on my form.
Please refer to the pdf version of the form below to review the form before starting to fill the answers

How do I achieve this?

Hi,
You can achieve that using this JotForm Widgets.
1. PDF Embedded Widget -- it displays a PDF form in your jotform that the form user can interactively review and use navigation options.
Example
2. Terms & Conditions Widget -- this widget is not necessarily for Terms, Agreements, or Agreement Conditions purpose only. You can also use this to have a PDF Link in the form that users will be able to click and use/read/review

Hi,
Thank you for your response.
I think the terms and conditions widget might work. But, I do have a couple of questions.
1) How do I remove the check box. I want only the link to show on my form like this.

2) How do I get the pdf link to my form. I was to download the pdf version of my form onto my computer, but not sure how to get the link and include it in the widget.

The PDF file should be uploaded somewhere. If you would like to upload it to your JotForm account, create a new form with a File Upload field. Then, upload the file and copy its link.
The PDF DOCUMENT link can be added via a Text field. Please see the next video:
